基于超声回波去相关成像的微波消融实时监测

摘要:

目的 为实现肝肿瘤微波消融的实时监测, 提出基于超声回波去相关成像的微波热凝固区检测方法。方法 共进行15例体外猪肝微波消融实验 (P=80 W, t=60 s) , 利用自编程序以2帧/s的速率连续采集微波热消融不同时刻下的超声回波信号, 分别计算每相邻两帧超声数据的瞬时去相关成像, 取所有瞬时去相关图像中各像素点的最大值构成累计去相关, 并进行对数压缩;设定阈值为-0. 4并结合多项式拟合技术, 在累计去相关图像中检测凝固区;测量并记录凝固区的长轴和短轴并计算其面积, 以消融结束后猪肝组织剖面内实际凝固区为金标准, 对凝固区检测算法的精度进行评估。结果 与B超图像相比, 瞬时去相关图像中热损伤组织和正常肝脏组织参数对比度增大, 可实时监测热消融过程中凝固区的变化。基于15例热凝固区的检测结果, 统计分析得出:累计去相关成像技术检测微波热凝固区的平均精度为88. 60%。结论 基于超声回波去相关成像技术可实现体外肝脏微波热消融的定量实时监测。

Objective To achieve real-time monitoring of microwave ablation for liver tumor, a detection method of microwave ablation zone based on ultrasound echo decorrelation imaging was proposed in this study.Methods In this study, 15 porcine livers in vitro ( P = 80 W, t = 60 s) were carried out in the microwave ablation experiments. The ultrasound echo signals at different times during microwave ablation were continuously acquired at a rate of 2 frames/s using the self-writing program. The instantaneous decorrelation imaging of each two adjacent frame ultrasound data was calculated respectively, and the maximum values of all the pixels in all the instantaneous decorrelation images were used to form cumulative decorrelation image, followed by logarithm compression. The threshold as-0. 4 and polynomial approximation were used to detect the thermal lesions in the cumulative decorrelation image. The long axis and short axis of the ablation zone were measured and recorded, and the area was calculated. The actual thermal lesion in the tissue section of the porcine liver after ablation was used as the gold standard to evaluate the accuracy of this algorithm. Results Compared with B-mode images, there was a larger contrast between thermal lesions and normal liver tissues in the instantaneous decorrelation image, which could be used to monitor the real-time change of coagulation during thermal ablation. The results of 15 cases of thermal ablation zonedetection showed that the average accuracy of the cumulative decorrelation imaging for detecting the microwave thermal lesions was 88. 60%. Conclusions The technique of ultrasound echo decorrelation imaging can achieve quantitative real-time monitoring of microwave ablation of liver in vitro.
