A mobile intelligent checklist system for stroke emergency and its application

目的 相比住院、ICU等医疗场景,脑卒中急救场景中面临的患者安全问题更加突出。智能核查清单是一种信息辅助工具,能够在疾病诊疗的关键环节动态生成患者个体化的核查内容。现有的智能核查清单系统大都需要医护人员在固定的工作站进行核查,这种核查方式难以适应脑卒中急救场景具有的时间紧迫、人员位置频繁移动、工作压力大和负荷重等特点。因此,有必要针对脑卒中急救场景中面临的患者安全问题及其特殊性设计一种智能核查清单系统。方法 通过调研、讨论确定在脑卒中急救场景下智能核查清单系统的需求;基于Drools开源规则引擎,设计了一种将临床规则与应用程序分离的系统架构;采用基于模板的方法动态生成清单内容,实现系统的灵活配置、快速部署。结果 针对脑卒中急救场景,实现了一种医疗人员能够在移动终端进行安全核查的智能核查清单系统;与临床专家合作,设计了静脉溶栓核查清单并进行初步测试,验证了系统的可用性。结论 移动智能核查清单系统可动态生成患者个体化的核查内容,为解决脑卒中急救场景中的患者安全问题提供了新的手段。

Objective The patient safety issues in the stroke emergency are more serious than hospitalization, ICU and other medical scenario. Intelligent checklist is an information assistant tool, which can dynamically generate individual checking content in the key link of disease diagnosis and treatment. Most of the existing intelligent checklist systems require medical staff to check at a fixed workstation, which is unable to adapt to the environment of stroke emergency due to time-critical, dynamical, high pressure and heavy workload. Therefore, it is necessary to develop an intelligent checklist system that can solve the patient safety issues and considering their and their particularity in stroke emergency process. Methods Through research and discussion, the requirements of the intelligent checklist system were determined firstly. Based on Drools (an open source rule engine), a system architecture is designed to separate clinical rules from checklist system. The content of the checklist is generated dynamically using a template-based approach that enables flexible configuration and rapid deployment. Results An intelligent checklist system for stroke emergency, which can be used on mobile terminal is developed. In cooperation with clinical experts, a checklist before intravenous thrombolysis is designed and a preliminary test was carried out, which verifies the availability of the system. Conclusions The mobile intelligent checklist system can dynamically generate the individual checking content of patient, which provides a new means to solve the patient safety issues in stroke emergency.
