基于高斯函数分解的妊娠期高血压患者脉搏波特征研究

Study on pulse wave characteristic in patients with HDCP based on Gaussian function decomposition

摘要:

目的 对妊娠期高血压疾病(妊高病)患者的脉搏波特征进行研究,以希望达到早期预测妊高病的目的? 方法 回顾性研究 143 名孕妇的脉搏波数据(其中健康孕妇 86 例,被诊断为妊高病者 57人),利用高斯函数将脉搏波进行分解并提取其特征参数,对妊高病患者和健康孕妇的脉搏波特征参数进行统计分析,对比其在不同孕周段的差异性? 结果 自孕 16 周开始,妊高病患者脉搏波的潮波均显著高于健康孕妇,妊高病患者的重搏波平滑且不明显;健康孕妇的第一个高斯波幅度( H1)均高于妊高病患者,第二个高斯波幅度(H2)均低于妊高病患者;在生理参数研究中,在孕 24 周以后,妊高病患者的收缩压显著高于健康孕妇(P<0.05),并且除孕 28 ~ 31 周之外,妊高病患者的舒张压显著高于健康孕妇(P<0.05)? 结论 运用 4 个高斯函数对妊娠期孕妇的脉搏波进行分解,能够得到妊高病患者和健康孕妇的脉搏波在各个孕周段内的差异性,并且这种差异性明显早于血压出现差异的时间,这对于提早预测妊高病的发生?促进母儿健康具有一定的临床研究和应用价值?

Objective To study the method of predicting hypertensive disorder complicating pregnancy( HDCP) by pulse wave, and to predict hypertensive disorder complicating pregnancy early.Methods Gaussian curve decomposition method was used to decompose and identify the pulse waveform and calculate its characteristic parameters. The physiological parameters and pulse wave characteristic parameters of pregnant women with HDCP and healthy pregnant women were analyzed statistically. Results From 16 weeks gestation, the pulse waves of pregnant women with HDCP were significantly higher than those of healthy pregnant women,the first Gaussian wave amplitudes ( H1 ) of healthy pregnant women were higher than those of pregnant women with HDCP and the second?order Gaussian wave amplitudes ( H2) were lower than those of pregnant women with HDCP pregnant women with HDCP were higher than those of healthy pregnant women (P<0.05) and diastolic blood pressures were significantly higher in pregnant women with HDCP except 28 - 31 weeks of pregnancy. Conclusions Using the Gaussian function to decompose the pulse wave of pregnant women with HDCP during pregnancy, we can get the difference between pregnant women with HDCP and healthy pregnant women in each gestational period, and this difference is obviously earlier than the time of blood pressure difference, which has certain clinical research and application value for predicting the occurrence of hypertension early and promoting maternal and infant health
