Objective A new method was proposed for calculating the tilt and decentration values of intraocular lens (IOL), and it was available to evaluate the effects of various types of IOL implantation and other conditions. Methods Based on anterior segment images of IOL from two patients, who suffer from the congenital subluxation of the crystalline lens after operation, the IOL tilt angle and the decentration value were obtained through curve fitting, image registration and basic geometric theories. Results The IOL tilt angles of 2 cases were 2.667° and 2.764° respectively, and the decentration values were 0.630mm and 1.397mm, which were all in the clinical ranges. Conclusion Geometry-based method to measure the IOL tilt and decentration values is reasonable and simple with a good repeatability, which is also available for other circumstances implanting with IOL.
