注射海藻酸钠复合支架修复RA膝关节软骨缺损的初步观察

摘要:

目的 探讨海藻酸钠支架复合组织工程化软骨细胞对RA患者膝关节软骨缺损修复的影响。方法 将8名RA

患者随机分为对照组4例和海藻酸盐支架治疗组4例。RA治疗组患者膝关节内注入1mL海藻酸钠支架复合组织

工程化软骨细胞,对照组患者膝关节注射等量的生理盐水。海藻酸钠支架复合组织工程化软骨细胞治疗一个

月后提取对照组和治疗组关节活检组织,应用Ⅱ型胶原免疫组织化学法染色和HE染色,通过计数Ⅱ型胶原免

疫阳性细胞数和光镜观察关节组织结构变化,检测各组RA患者的膝关节软骨缺损的修复情况。结果 组织形

态学结果显示,治疗组与对照组比较,治疗组关节浸润炎症细胞数量减少,炎症反应明显减轻,关节Ⅱ型胶

原阳性软骨细胞明显增高,软骨细胞有所增多。结论 海藻酸钠支架复合组织工程化软骨细胞对RA患者膝关

节软骨缺损有一定的修复作用。

Objective To explore the effect of cartilage defect repair using engineered chondrocyte composite through autologous bone marrow mesenchymal stem cell (ABMSC) cultured on the sodium alginate scaffold in rheumatoid arthritis (RA).Methods Eight patients with RA were divided into control group (4 patients) and alginate scaffold treatment group (4 patients) at random.Engineered chondrocyte composite with alginate scaffold were injected in treatment group and saline injected in control group.Knee biopsy tissue was extracted in treatment group and control group after one month of the treatment.Pathological changes of articular tissue were observed by HE and immunohistochemical techniques in treatment group and control group.Immunohistochemical positive stained type II collagen cells were counted and pathological changes of articular tissue were observed by light microscopy.Repair of articular cartilage defect was detected in control group and treatment group.Results Compared with control group,type II collagen positive cells remarkably increased and the number of inflammation cells in articular tissue reduced in alginate scaffold treatment group.The histomorphometric results showed that inflammation significantly reduced and the cartilage cells remarkably increased in treatment group.Conclusions Engineered chondrocyte composite with alginate scaffold can repair cartilage defect in RA.
